BLUE SKY HOSPICE  emphasizes palliative rather than curative treatment; quality rather than quantity of life. The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided. The patient and family are both included in the care plan and emotional, spiritual and practical support is given based on the patient’s wishes and family’s needs.

BLUE SKY HOSPICE affirms life and regards dying as a normal process. We neither hasten nor postpone death. BLUE SKY HOSPICE provides personalized services and a caring community so that patients and families can attain the necessary preparation for a death that is peaceful and comforting to them.

Hospice care is provided to patients who have a limited life expectancy who have made a decision to spend their last months at home or in a homelike setting.

Services

Skilled Nursing: Registered and Licensed Professional Nurses on staff twenty-four hours daily for consultation, education, scheduled and urgent visits to patient’s homes.

Medical Professional: Licensed Physicians available for staff consult twenty-four hours daily.

Therapy ServicesOccupational/Physical Therapists available if needed.

Dietary Services: Available as needed.

Home Health Aide/Nursing Technician Services: Available as a scheduled service.

Social Services: Licensed Counselor and Social Worker on staff as part of the multidisciplinary team to talk and provide assistance with social service, bereavement issues.

Chaplain Services: Regardless of religion Blue Sky Hospice is here to provide support.

Child Life Specialist Services: Available for families with children in the household.

Volunteer Services: Available for Blue Sky Hospice patients.

Our Team at Blue Sky Hospice

Those involved in the process of dying have a variety of physical, spiritual, emotional and social needs. The nature of dying is so unique that the goal of the hospice team is to be sensitive and responsive to the special needs of each individual and family.

Hospice care is provided through an interdisciplinary, medically directed team. This team approach to care for dying persons typically includes a physician, a nurse, CNAs, a social worker, a chaplain, a child life specialist and a volunteer.

Medical Directors

Our Medical Directors at BLUE SKY HOSPICE have overall responsibility for the care and treatment of patients and their families rendered by the hospice care team and will consult and cooperate with the patient's attending physician.  The Medical Director provides physician direction and guidance to the hospice program and its employees to assure the maintenance of quality standards of care for patients and families.
BLUE SKY HOSPICE Medical Directors are involved with:

Nurses

Our hospice nurses make regularly scheduled visits to the patient providing expert pain management and symptom control techniques. Throughout the time that the patient is under the care of BLUE SKY HOSPICE, the nurse keeps the primary physician informed of the patient’s condition. Nurses provide the complete spectrum of skilled nursing care and are available 24 hours a day, seven days a week.

CNAs - Certified Nurse Assistants
Under the supervision and delegation of the Registered Nurse, Our CNAs  provide assistance with the personal care of the patient.  Blue Sky Hospice CNAs assist patients with activities of daily living and maintain a patient care environment that is clean and orderly.

Social Workers

Social workers provide assistance with practical and financial concerns as well as emotional support, counseling and bereavement follow-up. They evaluate the need for volunteers and other support services needed by the family and facilitate communication between the family and community agencies. 
As interdisciplinary team members, BLUE SKY HOSPICE  Social Workers work with patients and families to develop plan of care goals and clinical interventions, educate, counsel, utilize community resources and coordinate services with other team members to optimize care.

Chaplains

Ministering to the terminally ill in hospice care, BLUE SKY HOSPICE Chaplains offer open, sensitive, and non judgmental presence accepting of different beliefs, cultures and values. Our Chaplains provide spiritual support to patients and families, often serving as a liaison between them and their religious community. They often participate or assist with memorial services and funeral arrangements.

Child Life

As a member of the BLUE SKY HOSOPICE team, our Child Life Specialists advocate for children and their families, and promote the philosophy of patient and family-centered care. Our Child Life Specialists design and implement program therapies and activities that help children and their families navigate the emotionally and physically demanding process of coping with death. 

Volunteers

Traditionally, volunteers have been the backbone of the hospice movement and are still a valuable part of the BLUE SKY HOSPICE program. Our trained volunteers can offer respite care for family members as well as meaningful support to the patient.  Our volunteers are at the heart of Blue Sky Hospice.
